Catholic Healthcare Community Services - Dementia Monitoring Program

This Home & Community Care (HACC) funded program assists people who have confusion associated with memory loss, dementia or a mental illness or disability, who live alone or live with someone who cannot function as a carer, in Ryde, Hunters Hill, Hornsby and Ku-ring-gai local government areas. It can provide flexible assistance with monitoring (eg: diet, nutrition and medication); management of risks; socialisation (eg: companionship, outings); maintaining living skills (eg: shopping, cooking and household tasks); arranging transport to appointments; coordination of other services as required. An Outreach Worker enhances monitoring through drop in and planned visits. The service values the person as an individual, their background, abilities and interests.
