ComPacks is a case managed package of care provided for up to 6 weeks after discharge from hospital. The length of service depends upon the amount of support required. This service is people who need two or more services in order for them to return home safely with appropriate care in place. Services available include; Domestic Assistance, Personal Care, Meals, Social Support, Transport, Respite Care, Small items of equipment and Home Modification and Maintenance. if ongoing assistance is needed after the six week period, other community services will be accessed
