Continuing Care Team - Gosford Hospital
The Continuing Care Nurse's role is to assess a patient who has complex discharge needs prior to going home from an extended stay in hospital and refer the patient to appropriate services within the local community. The Continuing Care Nurse also educates staff, patients and their carers on discharge planning issues.
