Future clients must have eshibit behaviours of concern and psychological symptons associated with dementia for EACH dementia package. An Aged Care Assessment (ACAT) is required to determine eligibility. Provide care packages to support people's choice to remain in own home. Packages have the flexibility to meet changing needs - up to the level of funding. A home visit is organised to discuss options and arrange suitable support. Do not provide personal response systems but can arrange at normal cost to client.
