Services available include: Community Nursing, Occupational Therapy, Speech Pathology, Counselling, Bereavement Support and Palliative Care. ACAT is now with the chronic & complex stream (west). Referrals are through Central Intake on 1800 013 101. Extended Care Services focus on support, care, treatment maintenance and palliation. They are coordintated from Lawson Community Health Centre and include the "Pines" and "Cottage" dementia and frail-aged Day Care Centres operating in the upper Mountains.
