Mosaic Community Care
Mosaic Community Care provides services to people with an intellectual and/or physical disability; also services people in the broader community.The following programs are for people with an intellectual and/or physical disability: Community Support Program - offering support to people who reside independently or with family in their own home. Residential Program - including 24 hour support, live in carer and co-resident models. Justice Program - offering outreach Support for individuals likely to be involved or are involved with the justice system. We help you decide what style of service and type of supports you feel will best meet your needs. You may wish to live independently with visiting support, or you may choose to share with another person/other people. We will assist you to plan your goals and care requirements, and if required, can assist you to prepare a COMBINED APPLIICATION PROCESS(CAP) submission to the Disability Services Commission.
