Southern Post Acute Care - Dandenong
Provides short-term care for up to 4-6 weeks following discharge from public hospital. Patient must be discharged from an acute setting e.g. Emergency Dept or an Acute bed. Referrals come from the hospital or a patient can self refer within 30 days of discharge. Have to have a Medicare card. Both children and adults are eligible. Patients must live in the following councils: Cardinia, Casey, Monash, Kingston or Greater Dandenong. They take referrals from Monash Medical Centres Clayton & Moorabbin, & Casey Hospital Berwick. Programs are as follows: Monday Mob - Activities for those with a physical disability who want to maintain their indipendence, physiotherapy is offered along with respite for carers (open for thos within the Bentleigh catchment area). Tuesday Treasures - Social & activities group for elderly persons who have a chronic illness, have been unwell, frail or socially isolated. Activities include gardening, craft works, indoor games and social exercises. Thursday club - Programs for adults with memory difficulties. My mum suffered a stroke in July and is currently in Woy Woy rehab centre. They’re hoping she’ll be home in 5/6 weeks. My dad is looking for someone to come in the mornings and evenings to help with mum. We are looking at between 4-6 hours per day. We’ll know more in 5/6 weeks as to the role itself but it’s standby assistance.
