Tweed Community Options (COPS) / Compacks - Tweed Shire
Case management as per HACC guidelines. Must be eligible for HACC services. Links are made into the HACC service system. Care plans are documented. Care plans are jointly written by client and the case worker. Provider of ComPacks, which consists of case management whereby the patient requires two or more services on discharge from hospital. Referral must come from hospital staff before the patient is released. Services last up to six weeks from time of discharge.

I am and have been struggling financially for quite some time now and don't like asking for help, with minimum help from centrelink due to the kids ages and not being classed as a single mum and only one shift a week at work my situation has become worse and am not breaking even to pay rent and my other bills.
