Case management as per HACC guidelines. Must be eligible for HACC services. Links are made into the HACC service system. Care plans are documented. Care plans are jointly written by client and the case worker. Provider of ComPacks, which consists of case management whereby the patient requires two or more services on discharge from hospital. Referral must come from hospital staff before the patient is released. Services last up to six weeks from time of discharge.
