The demonstration project encompasses three main areas: *Patients attendance at an accredited group Self Management Course developed by Kate Lorig, Stanford University, USA; * an allied health professional to work with GPs and patients in all facets of the project; *Joint planning, monitoring and evaluation by the patients, their GPs and project officers.
